How do you remove the "plastic quick connect cap that covers the INPUT port"? I tried prying it out with a screwdriver and an ice pick; and both just damage the plastic of the fixture but don't remove the cover.

Thank you for contacting us about this, I'm sorry it's giving you trouble.  There is a slot at one end of the plastic cap that covers the input port.  Insert a narrow (less than 1/8") screwdriver into the slot and pry the cap out.
