I installed-and programed this opener : 1) somehow the code self changed from 4 digits to 5 it repeated the last digit. Last night the code failed

It's always a four digit code. You can enter as many numbers as you want and it will work as long as the last four digits are the ones that you programmed. With out more information its hard to help with the code failing part. You might try giving Chamberlain a call.
