Does the rack come with all hardware needed to mount it and, will it fit on a 2016 Nissan Frontier?

The Quantum Rack does come with all necessary hardware to mount it to your truck. The rack mounts and clamps to the underside bedrail of the truck. You will have to have access to the underside bed rail and the lip on that bed rail. Most Nissan trucks do not have the capability to utilize the Universal Truck Rack.
