Was stationary but started moving then fell off

Had this installed when I got my bathroom remodeled as part of a matching set with the towel bar, and tp holder. When first installed the ring was stationary but recently started tilting from side to side. Today went to use the hand towel and the ring part fell off with the towel in my hand. I can see there's a screw hole in the ring part but due to the design you wouldn't be able to access the screw to replace it (I couldn't find it around the towel ring anywhere).
