4.5" is the outside diameter including the flange. Being aluminum, these are quite pliable. Not an issue for me. That said, the inside diameter was roughly 4". I ended up using a 4" hole saw to create the openings in my soffit. There are three "nubs" located equidistantly around the circumference of the inside flange that helps to lock the vent into the soffit. Personally, I added an 1/8" bead of white silicone to help adhere them to the soffit for peace of mind. All in all, it is a good product...
